PIAA Sports Physical Pre-Participation Information Packet & Instructions

FIRST TIME PLAYING A SPORT THIS SCHOOL YEAR

1. Physical Examination Forms (CIPPE Sections 5, 6, & 7) to be submitted to the High School Athletic Office.  Must be performed/dated on or after May 1st each year.
2. ImPACT Testing (details below) to be completed at home (once in middle school, once in high school, or any new athlete to East Pennsboro), with results only being viewable by EPASD Athletic Staff.  Results of completion will be entered by EPASD Athletic Staff as students submit registration requirements.

SECOND/THIRD TIME PLAYING A SPORT THIS SCHOOL YEAR

1. Re-Certification Form (CIPPE Section 7) to be submitted to the High School Athletic Office.

***Student-athletes who miss their respective registration deadline must sit out of the first day of practice or until the athletic office has processed all their registration documents.  Submission of registration documents does not guarantee completion.  Parents/guardians can verify registration completion by visiting their Rank One parent portal.

 

***The Athletic Office is currently accepting online registration, physical paperwork, and/or ImPACT Testing for Winter Sports.  Please drop off or e-mail your athlete’s legible physical paperwork to the Athletic office in the back of the High School during business hours.  For e-mail submission, please send them to the Athletic Secretary, Mrs. Wadlinger at [email protected].

 

All-Sports Parent Meeting:

We ask all parents of students who are involved in school sports(7th-12th Grades) to attend this meeting if they did not attend the Fall All-Sports meeting.  At this meeting, the Athletic Director will share information about academic eligibility, transportation, communication with coaches, and other athletic handbook procedures.  After the All-Sports meeting at 7:30 PM, each winter sports team will break-out into their team-specific meeting of which rules and expectations for that team will be communicated.  If you attended the all-sports meeting in the fall, we only ask that those parents to attend the team-specific meeting which will begin around 8:15PM.  Parent attendance is imperative toward helping us get the season off to a great start!  We appreciate your time.
