Documents Required for Enrollment

| Parent/Guardian's Current Photo Identification |
A current, government issued identification card including a photograph or signature with current address and a serial or identification number.
If applicable, the Pennsylvania Department of Transportation Change of Address Verification must accompany a Driver's License with outdated information.

| Current Immunization Record |
Document must include the child's full name, date of birth, provider information and immunization administration dates; English translation required (as applicable).

| Court Order for Custody | If applicable, a copy must be provided at registration. |
