Change of Residence (within EPASD)

It is required for each student’s record to contain the most current, accurate residency information.  If your student has had a change in address, please provide the following:

Documents Required for Change of Address

Change of Residence Form (within EPASD)

Email Student Registration Coordinator to request this form or obtain from District Administration Office

Proof of Residency

Parent/Guardian’s Current Photo Identification

A current, government issued identification card including a photograph or signature with current address and a serial or identification number.  If applicable, the Pennsylvania Department of Transportation Change of Address Verification must accompany a Driver's License with outdated information.

Court Order for Custody

If applicable, a copy must be provided.  


After the address is successfully updated, the Sapphire Student Community Portal will be updated with the new address and transportation information.  Transportation will begin within three school days after the request is processed.
